A ward member has asked me how to obtain a replacement Baptism and Confirmation Certificate for his daughter. She was baptized in a ward in another town in 1995. Even if I were to contact that ward, her records are no longer there and they would be unable to generate a new certificate. Her current ward can generate a certificate, but it will show her current ward information and the signature line will be for her current bishop. What is the recommended way for me to respond to this request?

That is correct. It may also not say who it was done by.

It's not quite a replacement, but that's all that can be done.
