Home Ministering Interviews
There’s been a lot of questions around what is a “Leader” in connection with Home Ministering Interviews. Who are the brethren wanting to do the interviews? Just the Elders Quorum and Relief Society Presidencies or are Home Ministering Coordinators encouraged to help with those interviews as well?

I believe a "Leader" is an elders quorum president or one of his counselors, or a Relief Society president or one of her counselors. I have always believed this, but the closest "official" reference I can find comes from the Enclosure to the First Presidency letter dated April 2, 2018, item 21, which begins:
Elders quorum and Relief Society presidencies keep a record of the interviews they hold with ministering brothers and sisters. Leaders indicate the month interviews are held and who participated in them.
It would follow that if the interview was not with a member of the presidency, it would not be recorded.
The First Presidency Letter and these instructions are available online at ministering.lds.org under Frequently Asked Questions.

As far as I can se3e it is the EP Presidency who interview the Priesthood and for the sisters they can have district leaders. The EQ secretary is going to be very busy
Re: Home Ministering Interviews
May an EQ Secretary supervise a ministering district and complete ministering interviews with the companionships?

According to the Additional FAQs dated May 11, 2018, the elders quorum president and his counselors conduct ministering interviews. Also, ministering supervisors have been discontinued and should be released, according to the same reference.

Does anyone know if LCR or MLS will generate an action interview list like it does for youth interviews in a bishopric?

I doubt it. But as the interviews are conducted and recorded at LCR (beginning July 1st *), the presidency should be able to readily see which companionships have not yet been interviewed during the quarter.
This is how they used to check to see if a family had been home taught. Now they will be checking to see if a companionship has had a ministering interview. I also expect that the secretary will have a large role in scheduling interviews for the presidency.
* [Edit: In one official source the date is July 1st. In another official source the date is August 1st. Take your pick.]

Are there any forms / reports that have been created for ministering, ie.. interview reports or visits made?

The Ministering page in LCR has a statement in a blue box at the top that includes "...Beginning August 1, 2018, leaders will have the ability to (1) account for interviews they hold with ministering brothers and sisters..."
Re: Home Ministering Interviews
Should the EQ presidency members be interviewed by other presidency members for their own ministering interviews? I'm trying to figure out how to divvy up the companionships. With home teaching I think the unwritten standard was your own companionship was in your own district (when no home teaching supervisors were called).
