Date descrepancies in records (geneology)

I have been doing a lot of research on LDS ancestors in the area around Lehi, Utah. I found a record that is puzzling, in the Membership of The Church of Jesus Christ of Latter-day Saints, 1830-1848 records I show that my 3rd great grand uncle is listed with Baptism Date: March 24, 1934 Endowment Date: September 10, 1934 Sealed to Parents Date: December 21, 1937 but he died 12 September 1911. I don't understand how this happened. All else is correct in the record, parents, birth and death locations, etc.

Any ideas?

Perhaps this will shed some light.
