I understand how to complete the form, but the question that I have is how long do I keep the hard copies once I have sent the report. I know that we keep the paperwork for actions that involve formal probation until the probation is lifted or changed, but how long do we keep the paperwork for excommunication and disfellowship. Do we shread the paperwork once CHQ receives the report of action. It's not very clear in the CHI (ch 10).

The form itself says that all paper and electronic records should be destroyed after the ward receives an updated membership record. It makes reference to page 122 of the CHI. I'd advise you to review the exact wording on the form and that handbook page (I know that page is in chapter 10, which you referenced, but in the context of the instructions on the form, the handbook instructions might make more sense).
