Limited Use Temple Recommends - Binder Paper Sheets

I was asked if we could order more of the binder sheets for the limited use temple recommends for the Youths. I was not able to find it on the lds store but heard something that this is all done online now through lds.org on LCR? Does the Bishop enter the limited use temple recommend information online and then just prints it out to give to the Youth to take with them to the Temple? How is the current process for limited use temple recommends?
