Form for travelling baptism?

A family is going out of state and will be baptizing their child while on the road. Where is the form that allows this to happen at the ward they will be visiting?

The child being baptized needs a Baptism Recommendation Form. You can get to it in LCR by calling up the child in the Member List, selecting the Profile, then the Ordinances tab, and choosing Add Baptism and Confirmation. This page will have a link to print the recommendation form.

If the person who will perform the baptism has a current temple recommend, he does not need a Recommend to Perform an Ordinance form. If he does not have a temple recommend, he will need the form from the location shown in the previous post.

You can find it on the website. I have also used this when I was traveling to Conejo last year. This is really a good thing.

I'd also recommend contacting the bishopric of the ward in advance, if you haven't already.
