I need to get the "Request for Ordinance Information" form in order to request a search for a member of my stake whose priesthood ordination wasn't recorded in the late 1970's. The only thing I can find is in the online store, where you have to order packs of 25 in hard copy. I don't see an option for a PDF version. Does anybody know how to get a PDF copy of this form so I don't have to order a pack of hard copies?

I too would like to be able to have a soft copy of this form. It is a little backwards that we should have to order these as printed material when you very rarely need to use them.
