Form for granting permission for stepparents to be sealed to stepchildren

I'm not sure where I could find this, but certainly it's on the interwebs somewhere. I'd like the form that a natural parent signs so that a stepparent may be sealed to a stepchild. Does anyone have a clue where this might be found, or specifically what it's called? Thank you!

Edit: Correction. The process is outlined in Handbook 1: 3.6.2. I don't think there is a form. The section cited outlines what has to be in the letter.
