patriarchal blessing recommends

We have a bishop that has gone online to submit a recommend to have a youth receive his blessing.
he filled it out and printed it for the youth to take to the Patriarch to which the patriarch had never seen one like it and was wondering if the church has new recommends.
this form has "Blessing Status" at the top then under that is "blessing recommend"
this is followed by the youths info etc.
Bishop wants to know if everything is online to get this boy a blessing or if there is a recommend like a temple recommend thats not online.
