The bishop starts a recommendation in the patriarchal blessing application. He should be able to click the submit button if he is logged in to his LDS account and the online submission is available in your area.

If he can't then I would call the global servic centre and ask them to check it out.

We received the letter in the last month and it covered the Europe Area. We show all general sessions of conference at our stake centre and 2 other buildings, the priesthood session is usually shown in 2 locations. In the UK any returned cheques are charged by the bank and the church covers the cost. It is a very rare event for those paying tithes and offerings and so the church doesn't worry about it.
