Patriarchal blessings are given to worthy members of the Church by ordained patriarchs. Patriarchal blessings include a declaration of a person's lineage in the house of Israel and contain personal counsel from the Lord. As a person studies his or her patriarchal blessing and follows the counsel it contains, it will provide guidance, comfort, and protection.
Filling out the Patriarchal Blessing Recommend form
MLS contains the Patriarchal Blessing Recommend form. From the Reports/Forms menu, select Forms. Select the name of the recipient from the list of ward members. Most of the relevant information required for the form is filled in automatically.
Membership records in MLS do not hold information about whether a person has received a patriarchal blessing or not. If a unit leader requires this information to be held it could done using a custom field.
Requesting a copy of a patriarchal blessing
A member may request a copy of his or her patriarchal blessing, or the blessing of a direct-line deceased ancestor, using the Patriarchal Blessing Request Form.
Clerks may be asked by a member to help them obtain a copy of a patriarchal blessing. This can be done by following the instructions given on lds.org for Patriarchal Blessing Requests. Members can obtain a copy of a patriarchal blessing for:

- A deceased direct line ancestor
Alternatively, you may call (800) 453-3860 x23581 and listen to a recorded message which has the same information. If you have any additional questions you may call (800) 453-3860 x22272.
