While on our Mission in San Jose, Ca. we baptised a lady & she moved & we moved & have lost contact with each other.

The church used to offer that, but I believe they've discontinued it. You're better off searching FaceBook, etc.
Have you searched the Help Center? Try doing a Google search and adding "site:churchofjesuschrist.org/help" to the search criteria.
