Insurance Claim for Injury

Does anyone have any experience filing an insurance claim for a person injury at a ward activity? We had a dance last Friday and our Primary President tripped over a bench in our cultural hall and dislocated her shoulder. So far their have been two trips to the ER, and a follow up doctors appointment yet to come, and the family in uninsured at the moment because of lack of employment issues.

I'm just information gathering here before I contact Deseret Mutual. I know a claim form needs to be filled out, although I have yet to locate that form on either the church website or Deseret Mutual's (you'd think they didn't want people to file a claim or something).

Anything else I should know before making going forward?

The church's activity insurance program is through Deseret Mutual. Information can be found in Handbook 2 (13.6.9) or by visiting their website: www.dmba.com/churchactivity

Personal Health and Accident Insurance

In many parts of the world, health and accident insurance coverage is available to Church members through employer-sponsored, personal, or government programs. Where such coverage is available, members are responsible to access all available benefits provided through it if they incur an injury during a Church activity.
Church Activity Insurance Program

In the United States and Canada, the Church Activity Insurance Program provides secondary medical and dental benefits and specific death and dismemberment benefits. This program is primarily designed to supplement, not replace, a person’s own health and accident insurance.
Members who plan, conduct, and supervise activities in these countries should be knowledgeable about the Church Activity Insurance Program, including its restrictions and limitations. The program is outlined in the Church Activity Insurance Handbook, which may be obtained by contacting:

Thank you for the information, although I had already read it.

More of what I was looking for is practical information from anyone who has been through the process that would be able to alert us to potential gotchas that handbooks don't cover.
