Finding forms online?

Finding forms online?

My bishop has asked me to determine if the form "Application to the First Presidency", used for cancellation of sealing among other things, can be found online. I believe it is form 35789. At least that is the number at the bottom right corner of the form. My bishop only has a couple of copies left.

If they are not online, how do we get more copies of this form?

It is located in MLS under Confidential Reports and Forms.

A quick search at http://www.ldscatalog.com revealed nothing even after logging in. However, as jdlessley pointed out, they have been embedded in MLS. That's probably where the "online" confusion came from. They can only be accessed on your unit computer and after logging into MLS, so you have to fill them out there.

Application to the First Presidency.

The Application to the First Presidency is now filled out online in Leader and Clerk Resources and is visible only to the stake president or the bishop, when they're logged in.
