Hello,
My Stake President asked me to track down the forms and proper procedures for calling a Stake Patriarch and counselor for the Stake Presidency. Are they on MLS? If so, how do we submit the completed form? via MLS, fax, etc??
Hoping to do this soon. Thanks for the help,

The form for counselor in the stake presidency is in stake MLS. The stake president (with your help, perhaps) fills out and signs the form. The form for a patriarch is not in MLS. I believe you have to request that form from Church Headquarters.
