Priesthood lineage

I have a member who ask me how to get a copy of his priesthood lineage. who does he call ???

I got this in an email several months back.
Beware, the voice mail tree options may have changed since then.
Brethren,

I discovered that the Church now helps supplying priesthood lines of authority upon request and at no charge. There appears to be three ways of accomplishing this:

Contact Member and Statistical Records by:
  1. Email: lineofauthority at ldschurch dot org with PLA in the subject line. In return you'll receive a form to fill out and return.
  2. Phone: 800-453-3860 ext 23500 then choose option 6, then option 3. After the message and tone leave a voice mail message with your fax number or mailing address. The form will be mailed to you. OR
  3. Phone: 800-453-3860 ext 23500 then choose option 6, then option 2. After being on hold (potentially for a considerable time), ask the attendant that you'd like a copy of your priesthood line of authority. They'll mail it to you.

Be prepared with as much of the following information as possible (your ward clerk MAY be able to help you with some of this information):
  1. Full name of the priesthood holder
  2. Date of birth
  3. Current Ward and Stake
  4. Elder ordination date
  5. Who performed the Elder ordination
  6. High Priest ordination date if applicable
  7. Who performed the High Priest ordination
  8. Address and Zip Code
It seems that many brethren do not have their priesthood line of authority for various reasons, or it may have mistakes. I find this very exciting and hope that it is beneficial to you.

Of course, this will only help if the membership records are updated and complete. We have 30+ MP holders in our ward where the "Ordained by" is blank.
Former membership clerk under 3 bishops, now on 2nd stint as executive secretary. Can I go back to teaching priesthood now?

I had a similar problem, but I went to each of them individually and asked them for the information. We have now reduced the number down to less than 10 that don't have ANY information except the ordination date.

Thanks for this info JBH001 I can't tell you the # of times I have been asked by members for their line of authority and I was stumped on how to tell them to get it. :)

We have similar problem with priesthood ordination on membership records in our stake, and ward. Many brethren do not remember by whom, let alone when, they were ordained. They either never received, or have since lost, any ordination certificate. I'm told there was a period before technology when this information was not reported on the paper forms, is this true?
Is there any way to recapture this information? It seems this information is critical to the future of the priesthood holder, given the emphasis on record keeping associated with the keys of the sealing power restored by Elijah.

There was a period of years (during technology) in the 1990s when the name of the person performing the ordination was not recorded. There is no way to recapture this information from the membership record system. If the information is located, it can be added.

There has been a concerted effort to gather the information through interviews, locating copies of ordination certificates, finding the yellow copies of ordination/approval forms in ward and stake clerks offices, and in other ways. When the information is located, it can be (and should be) entered into MLS, and will then become a part of the member's record.

Important, yes. Critical, no. At the link mentioned above, you will see that the priesthood authority of the member is still valid.
Because of missing information a priesthood line of authority may be incomplete. In such a case, your individual priesthood is nevertheless valid because:
  • Your stake president, through the priesthood keys he held at the time, authorized your ordination.
  • Your ordination was recorded in the official records of the Church.
And that is what is truly important.

What is the email address to send a request for lineage? I tried several and they don't work.
