Is there an official stake conference report form that needs to be completed before or after a stake conference?

There is a form that is sent by the President Packer's office to the Stake President. The presiding authority or someone he assigns fills out the document. It basically reports on the attendance at the meetings, and the events or impressions from the messages at the conference.

This is probably obvious, but the Officers Sustained Form for the stake should be filled out ahead of time for the first stake conference of the year and then read during that stake conference, allowing members to sustain the general officers of the Church and the local stake officers.

After the stake conference is over, the form should be filed. At the end of the year, it should be included in the Annual History for Stakes and Districts.

I did this but noticed that it did not fill in automatically the names and callings from MLS.

Anyone else experience this?

Two possibilities:

- The positions have not been filled in.
- Someone didn't use the built-in positions and created custom ones. Either because they couldn't find the built-in ones or they weren't happy with the title.

Is there a computer form of the Stake Conference Report or do we still have to use the stubby pencil format?

I've got a question about the stake conference report. Let's say a general authority visits your conference. At the conclusion of the conference, the stake clerk provides him with the statistical information for the top of the form. Now, who fills out the rest of the form regarding the summary and notes for the next visiting authority? Does the general authority fill that portion out or does the stake clerk do it?

Normally the visiting authority does it.
