How do I create a new record for readmission after excommunication

I am the ward clerk and I have a Baptism and Confirmation Record form for an ordnance just completed. At the top there is a box checked for Re-creation of a record. This is a baptism for readmission after excommunication. The website lds.org/lcr says if you need to create a record for the individual you must use MLS. In MLS there are only selections for "Child with record baptism", "Child without record baptism", and "Convert baptism". The convert baptism choice says its primarily for missions to use. Do I use the convert baptism one anyway? What is the procedure for recording this ordnance?

You let CHQ do it when they get the paperwork. Once the baptism and restoration of blessings takes place, their old record will re-appear.
