We have a brother in our ward who has been ordered by a court to perform community service. We'd like him to perform this service at a Bishop's Storehouse, but we're told by the storehouse that his bishop needed to complete a recommendation form before he could serve there.
Does anyone know what this form is? I've called the storehouse (who told me that yes, there is a form, but that they didn't know the name of it or where I could find one) and have visited a distribution center (who similarly wasn't familiar).
Any guidance or direction here would be great. Thanks for your help.

I would call back to the Storehouse and ask to speak with someone who is familiar with the form and can tell you exactly what it is. I believe there is a Bishop's form for Services (not the food commodity order). If the Storehouse can't come up with the answer, have your stake go up a level for you and speak with someone over the Storehouse.
