I can no longer find this form. I was able to access it and save progress on it in a password protected file in MLS, but it now directs me to the LCR system where it says access denied. I am the executive secretary and acting ward clerk (previous WC is now the Bishop and doesn't know how to use computers.)

You have to have the calling of Ward Clerk to see that part of LCR. Executive Secretaries don't have access. You'll either need to help the bishop do it using his login or be recorded as the actual Ward Clerk.
