Quarterly report not getting to church headquarters

Quarterly report not getting to church headquarters
A ward in my stake has submitted its 4th quarter 2015 report twice, but it still shows as not submitted in the quarterly report list. Has anyone else experienced problems with quarterly report submittal?

Is the clerk clicking the Save button or the Submit button? I can see that it is all filled out, but the ward clerk should be seeing a message at the top of the report that says "The report for quarter 4 of 2015 is past the due date of 15 Jan 2016.
