Dissolving a branch or ward
Is anyone aware of a form that must be completed in order to dissolve a Branch or Ward? I cannot find one on the website or any mention of one in the Handbooks. Any help on this would be appreciated.

Handbook 1: 9.1.2, 4th paragraph covers this. I suspect it's done on-line, at bl.lds.org.
Generally, dissolving a ward would require changing the boundaries of the neighboring surviving wards. So it makes sense it would be part of boundary realignment.

We had a Discontinued Unit 3 years ago and as Russell indicates, it was done as part of a boundary realignment.
