I member of my ward was baptized recently, and after the mission office created her record and we received it at the ward, I printed an Individual Ordinance Summary for her so she could register for an LDS Account.
A few days after I gave her the IOS, she called me to tell me that her birth date on the IOS was incorrect -- the day and month were correct, but the year was off by two. She couldn't register for an LDS Account because she was using her correct birth date on the web site, and it wasn't matching what was on her record.
I told her that when she registered for her LDS Account, she could use the (incorrect) birth date on her IOS, and I would update her record. I had one of my assistant clerks fix her birth date, and I gave her a new IOS. As of today, when I look at her Membership Record online in the Leader and Clerk Resources section, her recorded birth date is correct.
Today she called me to tell me that when she looked at her LDS Account Profile, the birth date listed there had changed, but not to what is currently on her Membership Record. Now the day, month, and year on her LDS Account are all incorrect.
Does anyone know how to fix this?

I'd suggest calling Local Unit Support.
Have you searched the Help Center? Try doing a Google search and adding "site:churchofjesuschrist.org/help" to the search criteria.

I would suggest that she go to LDS Account Help, which will take her to the LDS Account Support page. Near the bottom, she should click on "What if my issue is not listed here?" and then on the "Contact our support team" link.
When she checks the box ("I have reviewed . . .") a form will open up where she may describe her problem to the LDS Account support team and then submit the form.

If the birthdate on your LDS Account is wrong, then you need to see your ward clerk to make sure it's correct on your membership records and that you are using the correct MRN on your account.

i have accidentally made two different accounts because i had no idea how to recover the information for the other one and they both have the birthdate in and when i try to put my MRN in to the account settings it wont let me save
