Missing data in IOS and MLS

Some of our wards provide the IOS to members while they wait for tithing settlement. They are asked to check the information to see if it is correct. This year there is information missing that was there in previous years. Specifically the date and person performing the baptisms is missing. It is missing in MLS also. Does anyone know what has happened and how to fix it? Or has there been a permanent loss of data? Thanks

I'd expect the date to be there, but I don't think the "who" is kept at the CHQ-level. So a change in ward or a refresh could cause it to be lost. (And refreshes happen automatically with some software upgrades.)
Have you searched the Help Center? Try doing a Google search and adding "site:churchofjesuschrist.org/help" to the search criteria.

The person performing the baptism has always been missing unless the member was baptized in the unit and has never moved. Basically MLS stores that information when the baptism is recorded but it is never transferred to CHQ so when the record moves the information is no longer there. MLS has worked this way for a very long time.
