Sealing Clearance Web Form

I filled out a sealing clearance web form submitted to me by my Bishop last night and noticed a couple of disturbing items. First, the ward listed at the head of the form was wrong, it had the correct Stake but not the right ward.
Second, when I got to the end of the form, the Sub Button stated, Submit to Branch President, not Submit to Bishop. Is there anyone in Web Design monitoring this thread that knows of these bug in the Application to the First Presidency web area?

What do I need to be sealed to parents who have past but have there work done.

You need to have a current temple recommend.

As for the other steps, I'm not sure. This may be a case where you just need your family group sheets and go to the temple. I'd call the temple to be sure.
