Membership Information
How would someone go about finding out if they were still listed as a member if they haven't been active for a long time?

It's pretty simple. If you go to maps.lds.org and type in your address, it should show you which ward your address resides in. Listed there will also be the contact information for the bishop. I'm sure he'd love to hear from you. Your records may or may not currently be in that ward, but with a couple of clicks the bishop can see if they exist and request them.

Membership is not canceled for inactivity. Generally it's only by request or by major moral transgression. But you wouldn't be left in the dark about that. If you were never baptized, there's a possibility the child-of-record was canceled sometime after you turned 18.
