Disciplinary Action Tool
The online tool is terrible as it does not follow all the Church processes relating to council decisions which become recommendations in an application to the First Presidency. The tool does not allow this option to be selected, and a record of the council is required to submitted along with the application. MLS functionality for Disciplinary Councils has been inhibited so the paper forms are no longer available locally.

I never found that to be the case, I typically recorded the proceedings in the tool. Only thing I needed was the stake presidency notes on the decision reasoning.
