Baptism / Confirmation Ordinance

I received a request from a member to find out who baptized and confirmed her son. Neither she nor her son remember who performed the baptism and confirmation. Her son was baptized in 1981. I know we currently record this information in the MLS system. Is there anyway to retrieve this information from years past

You can try and call Local Unit Support, but that kind of information was dropped from the membership records back in the 80's. (The same time as the mag stripe temple recommends were dropped.) While there is a spot for that information on the certificate, it's not part of the membership record - at least not the permanent record.
Have you searched the Help Center? Try doing a Google search and adding "site:churchofjesuschrist.org/help" to the search criteria.

Although you enter this information in MLS it doesn't get transmitted to CHQ. If the record is ever moved out of your ward that information is lost. So it is unlikely that this information can be retrieved unless, as russellhltn mentioned, LUS has access to historical information that isn't available to us in MLS. But I wouldn't have much hope.
