Baptism: Recreation of Old Record?

We have a member who had her name removed from the records of the church and was recently re-baptized.

Should this be classified as a "recreation of old record" on the baptismal ordinance form, or a "convert" baptism?

I hope you went through all the steps listed in HB1, 6.14.4.

That is addressed in the Handbook section I referenced above. It is a "Re-creation of a record" -- Church headquarters will still have a record of the old membership record, and so they need to know that they need to look up the old record.
