Certificate of Insurance

Looking for some help, briefly: our youth org need to rent a local school for an activity (stake roadshow) they are practicing for (overbooked church buildings). The school district wants a Certificate of Insurance from any organization booking their facilities. So, how do we go about this?

I figure its authentication from something like "Corporation of the Presiding Bishop of the Church of Jesus Christ of Latter-Day Saints", which shows the church covers the members during their church sponsored activities.

I checked out the policies in handbook #2 about Church Activity Insurance Program, but all of the information from Deseret Mutual was about claims and requesting assistance, not about documentation showing the church insures it's members official activities. I haven't called them yet (business hours), its convenient to post now while it's on my mind.

The documentation for the Church Activity Medical Assistance program can be found at http://dmba.com/churchactivity . In particular, if you look in the CAMA Handbook, you'll see on page 2 in bold type, "CAMA is not insurance" and "CAMA is not liability insurance." So I don't know if you're going to be able to get a Certificate of Insurance.

It is interesting that Handbook 2, Section 13.6.9 (which links to the DMBA site referenced above) does call this program "Church Activity Insurance Program," whereas DMBA is very careful to declare that it is not insurance. In any case, both that handbook section and the DMBA site provide contact information where you can follow up on this.

You need to contact church headquarters and they will be able to help you. I will send you a PM with the form that I got when I needed one about a year ago.

Thanks everyone for the replies, which were spot on. I talked to both Deseret Mutual and the Risk Management department. DM emailed a form that verifies they will step in on behalf of church members and help with medical expenses beyond personal insurance coverage. Risk Management emailed a request form that details the event for the church to review. Hopefully this thread will quickly point others in the right direction.

This thread now goes to an error page. Does anyone know where to find this certificate of insurance for a stake activity? New stake clerk here, so any help is much appreciated. This is for a youth trek activity.

With the exception of the link to Handbook 2, all other links seem to work for me. Since I'm just a lowly assistant stake clerk outside of this forum, it should work for you. (At least it should after your calling propagates to all the systems.)

What do you mean by "this thread"? I clicked on every link in posts this topic (two to DMBA, one to Safe Church Activities on LDS.org, and one to Handbook 2), and every link worked, except the Handbook 2 link, which I have now fixed. Could you be more specific about which link is not working for you?

Apparently this process has been moved online. In Feb 2015 I was in the same situation as the original post (had to book a local school because a stake activity bumped our gym reservation and no other church gyms were available in our stake). I called risk management and they directed me to an online application process:

1. Visit this URL - https://insurancecertificates.lds.org
2. Click on Certificate of Insurance
3. Use your lds.org username and password to log in and complete the application.

Take note of your certificate request number and you will receive an email confirmation.

I am the housing coordinator for the California Santa Rosa Mission. We have an apartment complex in which we are opening a new apartment. They are requesting that the Memorandum of Insurance document that we gave them have the apartment address on it officially from you. Is that possible?
