When you show the list of callings for an organization, there are two sections: the leadership section (typically that's the presidency), and below it the other section (for teachers, etc.). If you check "Is leadership" then that calling will appear in the upper section. That flag is also available on the MLS Export file for organizations.

I don't know of any effect that flag has on LDS.org.

On the LDS.org directory, "leadership" can see profiles that have been set as private. I was curious if the "is leadership" flag linked into that, or if it's just standard leadership positions.

I'm not sure where the term "leadership" is used in relation to the Directory permissions, but in any case, it has nothing to do with the "Is leadership" flag. For one thing, you started off talking about custom positions, and custom positions never confer any online permissions. And for another, the set of permissions is for a specific set of callings, which has no direct correspondence to the callings in the MLS "leadership" section (although there's obviously a lot of overlap).

I don't know of any effect that flag has on LDS.org.

It doesn't have any effect outside of MLS. Although it might control which organization group that those callings show up in. Since the presidencies are now separate 'organization groups' within the ward, I'm guessing that it would show up in CDOL and Clerk Resources under the presidency group instead of the general list.
