Accessing Home Stake & Assigned Stake
I was recently called to serve in a YSA Stake. My LDS tools now gives me access to the YSA Stake & Ward information. I would also like to have access to my home Stake & Ward Information. How do I do that?

You posted in the Directory forum. Since the issue and topic deals with LDS Tools you may get some response if you tell us what LDS Tools operating system you have. Then we can move the thread to that forum. The response may be different for each operating system's LDS Tools.

Note that if the YSA ward moved your records instead of creating an out-of-unit record for you, you will have no access to your "home".
Just as a test, what happens if you log into the directory on lds.org? You should see a way to select both units unless your record was moved.

Have you searched the Help Center? Try doing a Google search and adding "site:churchofjesuschrist.org/help" to the search criteria.
