Bank of America
the local Bank of America - where many of our units deposit - has been trying to get return phone call from posted numbers on the deposit account (i.e. suspect at HQ in SLC) - no reply. Bank manager tracked down our Stake President and called. Said that if a deposit slip or paper work is put in top with Cash, they have to open it at local bank.. There would be a $55 fee per bag opened. stop putting the slips in top with the cash....please. Anyone else?? seems instructions for slips / bags needs to be updated quick RW

Our units deposit at Wells Fargo, and we've received no such complaints from the bank.

Sound like something you need to call LUS about and let them solve the issue with the bank. Maybe the local manager does not understand the agreement the church has with the bank.

There should be one deposit slip with the coin and a copy with the checks. The purpose of the dual part deposit bag is so that the coins can be counted locally and the checks processed at a central location. If you do not put a deposit slip with the coins, you risk their disappearance.
No one else in town uses the dual pouch deposit bags the Church uses, but the local BoA has figured out the system and we have not had any complaints.
