My new address is correct in the directory, however, the map displayed is of my old address a thousand miles away... how do I correct this?

Ask your new ward clerk to move your household marker on the Maps application on LDS.org to be in the correct position.

Another option is to correct the placement of the household marker yourself. When you click on your household marker, the information box that pops up should have a Move option in the lower left corner. Click there and you will be asked to position the map such that the new location for the marker is visible. Then click on the START MOVE button. The cursor will change to a cross-hair, which you use to click on the desired new location for the marker placement. Then click on the SUBMIT button to complete the procedure.

That may or may not be available. If a clerk at some point moved it, it won't be available.
