Is the Travel/Phone Reimbursement form only available as a paper form?
From the thumbnail image at store.lds.org, it looks like a fillable PDF, but there is no way to download it.

I received the single copy of the form I had ordered. It is a multipart form. I only ordered one because I was afraid "one" meant a pad of 100, but not so.

My FM has confirmed that they use the standard IRS rate of 56.5 cents per mile.

I just wanted to let you know that you can now download the Travel/Phone Reimbursement form from the product page on store.lds.org.

Also, addressing your second post, you can see if an item comes in a package of more than one if you actually select the item. For instance, on this page: http://store.lds.org/webapp/wcs/stores/ ... -1__195637, if you select "English" it will say, in red, next to the quantity box, if more than 1 comes in a package. This pamphlet shows that it comes in a package of 25. The form shows that it is a 1 each item.
