Are you looking for a copy of a patriarchal blessing? A certificate showing that an ordinance took place? A copy of your Individual Ordinance Summary?

Your clerk can print the latter two. You can request a copy of your patriarchal blessing at LDS.org by clicking on My Account and Ward near the top right, then Patriarchal Blessing at the bottom of the left column in the drop down menu.
