Convert branch to a ward

I am the stake clerk and we just converted a branch to a ward but I can't find anywhere to make the change in the LCR. Is this something done at church level and if so what needs to be done from our end to make it happen? I also need to change the branch president to bishop and I'm assuming I have to wait until the branch is converted. Any help would be greatly appreciated.

I'd look at Handbook 1 9.1.2. To make the change, a form has to be filled out and submitted to CHQ.

Your stake president needs to notify Church headquarters that the ward has been created, and that the bishop has been ordained and set apart. Once that notification is sent to headquarters, they will make almost all of the changes for you.
