Patriarchal Blessing Recommend issues

I am the High Councilor assigned to Technology. I just got off the phone with a bishop who is reporting a bug that consistently prevents him from being able to add a recommend to the Patriarchal Blessing Recommend system.

His description of the problem is as follows:

On the first screen there are 2 boxes, one of which is "Add Recommends." From there he gets a screen that shows who already has their recommend with another option to "Add Recommend." From there he attempts to select a ward member. As he begins typing the name of the member, he gets a dropdown (autosuggest) list. When he clicks the name of the member he wants from the dropdown list, it never autofills the input. If he attempts to type the full name without clicking the autosuggested name, he is still unable to continue.

Please advise.
