PDF of Unit & Leader Change Request

PDF of Unit & Leader Change Request
I need to submit a Leader & Unit Change Request for a couple new bishops. I am out of state and cannot submit it through MLS (and most of the stake presidency/exec sec are gone as well). I happen to be in SLC and was thinking of dropping off a hard copy at the Global Service Center while the kids wander around Temple Square. I can't find a PDF of a blank form. Does anyone here have one they can send?

I am not aware that there is a form any longer. Normally you would submit the change through MLS, And if you cannot do that, you would print out the current information from MLS, mark it up with the new/correct information, and then mail or fax that form to Church Headquarters, the way the manual system was handled in the past.
One of the challenges is that even if you walk into the Church Office Building, they have no way of verifying who you are and that you have authority from the stake president to change the information. He would normally sign the form anyway. Submitting through MLS serves as a way to verify that the information is coming from an authorized source.
You mentioned that "most of the stake presidency/exec sec are gone as well". This implies that they are not all gone. There must be at least one who could make the change in MLS, even if you had to walk him through the process over the phone. I think that would be simpler than trying to walk paper into Headquarters.
