Where is the form for Restoration of Blessings in MLS?

We have a member who was recently re-baptized - about a year ago. He is getting ready to receive Restoration of Blessings. Where is the form for this?

It's in Confidential Reports and Forms, and is called Application to the First Presidency. The form contains detailed instructions as to how it is to be filled out and submitted.
