A former member of the church was recently re-baptized into a ward.
How should the clerk record this? He just filled out the paper baptism
form as there did not seem to be any way to do this in MLS.
This member does not wish to even mention the current spouse on the
record (pending divorce), and the clerk did not press the issue. How
should this be handled?

Do not use the paper baptism form. Use the form "Baptism Record for Readmission" found in MLS under Confidential Forms. That form needs to be accompanied by some forms that the stake clerk and stake president will have to fill out, so coordinate this process with them.colbyj wrote:A former member of the church was recently re-baptized into a ward. The form does ask for that information, so for any exceptional circumstances, you should counsel with your bishop. Another Rebaptism Question
Do I create a new Baptismal Certificate? Or is the orginal certificate sufficient?

Once the readmission process is totally complete (and it may involve other steps that can take a year or more), an updated membership record will be sent automatically from CHQ that will have the original baptismal date. So the original should be sufficient, although I know of no reason a new certificate with the new date could not be issued in the meantime if the member would like one.LTORDUSA1 wrote:Do I create a new Baptismal Certificate?
