Bishop, Stake President, and the Office of the First Presidency approval. CHI Book 1, pg 145. Pages 145-46 list the three-month rule and some other exceptions that do not require First Presidency approval, and explain what mkmurray said, that records should be sent to CHQ (or the area office) when local efforts to locate the member are exhausted.
