Annotation of records by stake president

Where can the stake president add an annotation onto a record? Can it be done on LDS.org?

If the annotation is the result of a disciplinary court then it should be handled as part of that report. Otherwise I believe you have to write a letter to the Office of the First Presidency describing the situation. I can't provide a specific reference but check in the section of Handbook 1 concerning church discipline.

It wasn't part of a disciplinary council. There is something from the past, pre baptism, that should be annotated on the record

The stake president should review Handbook 1, 6.13.4.

Thanks for the info, I hadn't thought of looking at the disciplinary section as a council hadn't been held.
