Baptism forms
Who fills out the paperwork for a re-baptism (member was Ex-communicated and is being rebaptized). and Where do we find it?

Confidential Forms
The forms are in MLS under Other Resources > Confidential Forms. The Confidential Forms page in MLS has good information about precisely which forms you need -- review that carefully. Generally the Stake Clerk fills out and signs the forms, under the direction of the Stake President, who also signs them.
