On-line Donation appears but member never made it

On-line Donation appears but member never made it
We have a member that sees a $16 donation only in the on-line donation on their lds account. It says "Ward" so it should be in MLS, but it is not. There were no other $16 donations on that day. They claim they have never made the donation, their bank account is also not set up and nothing appears on their bank statement.

No, it just appeared when they first logged on.

I would suggest they or the ward clerk call the Global Service Desk.

You might want to have the member confirm the MRN on their lds account. Failing that, it's probably a wrong MRN attached to whoever is making that donation.
Have you searched the Help Center? Try doing a Google search and adding "site:churchofjesuschrist.org/help" to the search criteria.
