To use the Patriarchal Blessing Recommend form in MLS, I first need to have our Stake Patriarch loaded. How do I do this?

I'm not sure what you mean by "loaded". The Patriarch Information section of that form has a selector button that will allow you to choose any high priest in your ward. MLS will fill in his name and other details. If he lives in another ward, you'll have to fill in the information manually.

Then you fill in the rest of the information.

To save a little bit of time, some people do that first step and then save the form. Then they can start with the saved form each time they want to fill out a new recommend form. See Patriarchal Blessing Recommendation form for details.

Is it really only if he lives within the ward? The stake can specify who the patriarch is the same way they specify stake presidency and bishops in the Leader and Unit Change Request Screen. This is how I would assume one "loads" the Patriarch. I guess I just assumed that information automatically fed back to the units. I know from a recent conversation with LUS that is how it is supposed to work and they pushed down a calling refresh to one of my units that was not seeing our stake's patriarch. I don't know if that fixed the problem, but I haven't heard any more complaints from the unit about it.

It was definitely the case a few months ago that wards creating a Patriarchal Blessing Recommend form could not select the patriarch unless he lived in their ward. That was the main point of the Patriarchal Blessing Recommendation form thread.

Perhaps it has been fixed. The fact that LUS had to push a calling refresh down to one of your units would seem to be an indication that it's not working in every case. But if it works for most units and a calling refresh will fix it for the remaining units, then that is a nice improvement.
