Trip Report for traveling out side the stake boundries
Is there a form to fill out for travel outside the stake boundries. The Stake has told us we need a Trip Report filled out to go to Baptisms for the Dead in our Temple District but outside of our Stake Boundries.

According to Section 13.6.24 in Handbook 2: "Leaders fill out an Activity Plan form for activities that involve travel outside the local area." The form is the Activity Plan form.
Note also that the guideline is not "outside the stake boundaries" as you mentioned, but "outside the local area." For many stakes in Utah, traveling two miles could be outside the stake boundaries, but would not require an Activity Plan. For stakes that cover a large geographical area, a ward may need to fill out an Activity Plan for a long-distance trip that is still within the stake boundaries.
