Service missionary recommendation form

Service missionary recommendation form
Our bishop has a senior couple that he is recommending for a part-time church service mission. Is that handled through the online missionary recommendation system or is there some other form he needs to fill out?

The online missionary recommendation system is used only for full-time missionaries. For part-time service missionaries, there is a separate form.
