Trying to find the " Report of Administration action " form

I'm hoping someone can help me find the " Report of Administration action" form.

I'd imagine it would be in LCR, but one would have to be a bishop, ward clerk, stake president or stake clerk to access it. It's not available to councilors or assistant clerks.

If you have the right calling as mentioned by russellhltn you will probably find what you need in LCR under "Membership". Look for a "Confidential" heading there.
