Handbook 1, 13.7 covers the responsibility to create an annual history. I no longer have access to Handbook 1, but I'm confident it still refers to that form. You can find the form, guidelines, and many other helpful resources at https://www.lds.org/annualhistory.

The reference aebrown gave is accurate. The form is specifically mentioned in the third paragraph.
